Benefits Summary

Flex Benefit Plan encompassing Health, Dental and FSW
Health Insurance - Pre-tax payroll deduction for individual coverage, two-party coverage and family coverage. Flex credits (employer contributes each pay period) to assist employees in purchasing coverage. CMH offers Blue Cross Blue Choice with an opt-out. Insurance is effective the first of the month following your initial 60-day waiting

Dental Insurance - Dental plan with minimum deductible and co-pay based upon service. Routine cleanings are covered at 100% of reasonable and customary. Employee may use their dentist of choice. Based upon reasonable and customary, if your dentist charges higher than reasonable and customary you may be balanced billed.

Staff Life Insurance - Valued at $25,000, accidental death and dismemberment.

Supplemental Life Insurance - Available for spouse and children through payroll deduction.

Flex Spending Account for Dependant Care and Health Care - Payroll deduction on a pre-tax basis.

CMH Pension & Savings Plan - Eligible on January 1st following date of hire if you work at least 1000 hours a year. You are vested at 5 years provided you have worked 1000 hours each year. For employees working in a .5 part time status and above, tax sheltered annuity (TSA) contributions may be made on a pre-tax basis with CMH. CMH will match 50% of your contributions up to 4% of gross pay to your TSA, for a maximum match of 2% of gross pay. (Must be employed here 90 days prior to open enrollment.) Open enrollment is held quarterly.

Personal Leave- 16 hours of personal leave effective after 90 days of employment.   If hired after July 1 leave is pro-rated to 8 hours.     

Part Time Leave - Accrue leave on a pro-rated basis to be used for sick or annual leave. Must be employed six (6) months before leave is available.

Bereavement Leave - 8 hours.

Credit Union - CMH is a member of the Cedar Point Federal Credit Union. Employees are eligible to join via payroll deduction. The Credit Union office is located in Prince Frederick in the Prince Frederick Shopping Center.

Paid Meal/Break Time - During the course of an eight (8) hour shift, one (1) fifteen (15) minute break period and one (1) thirty (30) minute meal time. On twelve (12) hours shifts, two (2) fifteen (15) minute breaks and two (2) thirty (30) minute meal times.

Discounted Meals - Excellent meals at reduced rates in the cafeteria.

Pharmacy Privileges - Prescriptions filled at reduced rates. Payroll deduction is also available.

Hospital Recognition Program - Annual Awards Banquet and recognition vary depending upon achievement. Customer Service Program included in Recognition Program.

Social Functions - CMH sponsors several social events during the year (i.e., family picnics, trips to Atlantic City, dances, etc.)

Day Care Center on Site - Subsidized Infant/Toddler and Pre-School Child Development Center.

Direct Deposit - Available to any bank of your choice. DIRECT DEPOSIT REQUIRED OF ALL NEW EMPLOYEES. All payroll statements will be mailed.

U.S. Savings Bonds - Payroll deduction availability.

Employee Assistance Program - Eight (8) free counseling visits to assist you during your time of need.

Long Term Disability - Automatically carried 0.5 or greater

Short Term Disability Plan - Disability for up to six (6) months once qualifications are met. Fully paid for by employee.

Family Type Atmosphere - Friendly environment, cohesive workers and caring staff.

Worker's Compensation & Liability Coverage - All employees are covered under these programs.

Weekend Differential - All staff employees scheduled to work the weekend will receive a weekend differential. Refer to the policy for amounts.

Wellness Programs - Eligible employees can participate in wellness programs at reduced rates. On-site fitness center and massage therapy available for a nominal fee.
