Q: Why is Calvert Memorial Hospital starting this program?
A: Employees are one of CMH’ s greatest assets and we have decided to take a vested interest in designing a program, which promotes healthy lifestyle behavior, encourages employees to take care of themselves and personal responsibility for their health. Improving employee’s health creates a healthier workforce, helps reduce the rising cost of healthcare and improves our quality of care.
Q: Will this program be available to all employees?
A: This program will be available to employees that work at least 400 hrs/year. This is at least one full day per week.
Q: What do I have to do to participate?
A: The first step is to read about the Health Risk Assessment and determine whether you will complete one.
Q: What is a Health Risk Assessment (HRA)?
A: A Health Risk Assessment (HRA) is a computerized questionnaire that asks questions about your health, lifestyle behaviors and a section on biometric testing such as height, weight, body composition, blood pressure, resting heart rate, cholesterol, glucose and flexibility. The HRA utilizes all this information to provide you with a Personal Wellness Profile (PWP). When you complete a HRA, you will receive a $20.00 credit on your Wellness Advantage Debit card.
Q: Why should I do a HRA?
A: The most important reason to do a HRA is it provides you with a Personal Wellness Profile (PWP). This PWP is an educational report designed to help you evaluate your current lifestyle, identify health risks and decide where and how to make improvements. The choices we make each day regarding lifestyle behaviors all have a profound effect on how long we live and the degree of health we enjoy.
Q: What if I don’t want to do a HRA?
A: Completing a HRA is voluntary and you are not required to do one. However because your health is important and CMH wants you to take an active role in your health, you will receive $20.00 on the Wellness Advantage debit card just for completing the HRA. In addition, if you are a benefits eligible employee you will also receive a reduction on your medical insurance premium.
Q: Where do I go to complete a HRA?
A: To complete a HRA you can go to the KeepWell Fitness Center. During the month of July we will have designated times for employee to complete their HRA, (see attached). If you are unable to make it during the scheduled times, you may call the KeepWell Fitness Center staff and make a one-on-one appointment.
Q: Can I complete a HRA after open enrollment?
A: A HRA may be completed at anytime during the year, however the medical insurance premium reduction will not be applied to your current medical premium deductions. That will only be done at the beginning of each plan year in July.
Q: Do I have to do a HRA every year?
A: The HRA will be part of the annual medical insurance enrollment process and a reduction will be available to those employees who complete an annual HRA. In addition, if you complete a HRA annually you will receive a report comparing your HRA from last year to the current year.
Q: Will my personal information be kept confidential?
A: Absolutely. Each employee that participates in a HRA will be given a personal ID number and all reports will only include that ID number, no names. The Director of KeepWell will keep a cross reference list of employees names and ID numbers however this information will not be shared with Human Resources nor employee supervisors and will only be used in the event an employee can not remember his/her ID number.
Q: What is a Personal Wellness Profile (PWP)
A: The PWP is your personal results from the health risk assessment (HRA) and identifies current health status and areas for improvement. It also assesses your risk for some key diseases like heart attack, diabetes and cancer. It helps you set personal wellness goals to increase the years and quality of your life.
Q: How does this Program affect my medical insurance premium?
A: The medical insurance premium has a three-tier rate structure.
All employees start at the Top Tier /Rate. If you do not do a HRA you will remain here.
If an employee chooses to complete a HRA he/she will Receive the first reduction or discount and move down to the Middle Tier/Rate. If an employee has no health risk factors associated with high body composition, smoking and/or are under the care of the physician for managing their health risks such as High Blood Pressure, High Cholesterol, Diabetes and maintaining a “Healthy Life Style” status, they will receive the second reduction and move down to the Lowest Tier/Rate. (The premium rates are attached for fulltime and part-time).

Q: What happens if I have high health risk factors?A: Employees who have health risk factors associated with high body composition, smoking, high cholesterol, high blood pressure or diabetes will have the opportunity to enter into the Improved Health Risk Factor Incentive program. An employee must complete a (HRA) to qualify for this program. If an employee is found to have health risk factors outside of the “desirable” status, they have the opportunity to receive a $25.00 per quarter credit for each program criteria on their Wellness Advantage Debit card if they follow each program criteria. For more details on program criteria see Personnel Policy & Procedure P3-17.
Q: What can I use my Wellness Advantage Debit card for?
A: The Wellness Advantage Debit card can be used to cover co-pays, dental fees, over-the-counter drugs, and prescriptions, massage therapy (with physician referral), acupuncture and any other expense covered under Health Spending Account regulations. Call Human Resources for a complete listing of qualifying expenses.
Q: Do I have to complete a HRA to join the Fitness Center?
A: No. You may join the fitness center at any time and it does not require you to complete a HRA. However we would like to encourage all employees with at least 400 hrs/year to take an active role in learning more about their health status.
Q: Will employees still receive 50% off KeepWell Classes?
A: Yes. Employees who complete a HRA and work at least 400 hrs/year will be able to receive a 50% discount on classes and programs with the exception of alternative therapies such as full-body massage, acupuncture, hypnotherapy, etc., employee will receive a 25% discount.
Q: How does the wellness incentive plan work?
A: There are several different incentives available through the employee wellness program. They are listed and explained below:
HRA Incentive:
All employees who work at least 400 hr/year and complete a HRA will receive a $20.00 credit on a Wellness Advantage Debit card, be eligible for 50% on KeepWell classes and 25% discount on Healthy Alternative therapies.
Employees who have medical insurance with CMH will receive a reduction on their biweekly premiums and will be in the middle tier/rate. Employees who do not have health risk factors associated with high cholesterol, high blood pressure, high body composition or smoking, will be eligible for an additional reduction on their biweekly premiums and will be in the lowest tier/rate.
Improved Health Risk Factor Incentive:
An employee must complete a HRA to qualify for this program. $25 per quarter will be given to employees who improve their health risk factors by making significant progress in moving toward a “desirable” status in one or more of the five Categories listed below. See Personnel Policy P-3-17 for complete details.

On-site Fitness Center:
All employees are eligible to join the fitness center at a discounted rate of $25.00/month or if they sign up for a six-month membership they get a discounted rate of $20.00/month. When employee members of the Fitness center workout 2-3 times per week every week during each month, they will also receive a “fitness bonus” of $5.00 credit toward their membership fees. This credit can only be applied toward future membership fees. Q. Where can I find Personnel Policy P-3-17?

Q. If I have questions regarding the program whom do I call or see?

Q. Will I be paid for attending the informational sessions and the Health Risk Assessment time?
A: Yes, you will be paid, however it will not be as overtime. Please clock in/out either through Kronas or your time card. The Wellness and Human Resource Department will make every effort to be flexible in making time available to you to complete the HRA. |
