CMH staff are available to address concerns related to managing your illness or condition and to assist you and your family in preparation for discharge. Social workers can provide counseling for the personal, social, emotional and financial stress you may experience as a result of your illness or injury. Substance abuse services are also available. Working with your physician, case managers can provide information about healthcare agencies and other services available to you when you go home.
