Transitional Care Unit

When you exit the elevator on the fourth floor of Calvert Memorial Hospital and enter our Transitional Care Unit, you'll find it hard to believe you're still in a medical facility. The 16-bed unit brings an innovative approach to patient recovery, filling the gap between shortened hospital stays and home. Patients find a "home away from home" with the unit's warm, inviting surroundings and personalized care.

Every patient's plan of care is customized to their specific needs, receiving more medical and nursing care than is practical to provide at home. The patient remains under the care of the same physician. Patients and their families are involved in setting goals and learning the skills needed prior to going home. Skilled nursing services such as intravenous therapy (IV) pain and wound management are routinely at hand. Rehabilitative specialists in physical, occupational, speech, recreational and respiratory therapies coordinate with nurses and physicians as a team.

Patients usually stay in transitional care from one to two weeks. For comfort, patients wear their own clothes -- jeans, dresses, shorts, pants -- whatever they choose, just like at home. Rooms are beautifully furnished with warm patterns and colors. Days are filled with individual and group therapeutic, recreational and educational activities.
